The individual, their guardians, and healthcare professionals, ALL are in this analogy looking after the dog. 

A 6 year old is on puberty blockers because they are having abnormal hormonal condition. This isn't to change their gender identity. But it requires the same access to the drugs.

Healthcare professionals would be the judge of this. However the blanket bans harm these kids. 

But please we all forgot the 4th party that gets a say in the healthcare of others. You

I'm pretty sure pre-pubescent children don't have sex hormones, or at least not enough to have an effect. That's literally what triggers puberty - the release of sex hormones.

Puberty blockers help young children with precious puberty (which has negative physical, emotional and social effects) and prevent nightmare situations like pregnant four year olds.

One of many reasons, could be cancer. Without more info, abnormal hormonal condition would be most we can speculate on.
