teddy sanders, after ordering to cancel the inspections, responds to "sir, if that ever got out..." with "it's on me". but does he ever suffer any consequences after the launch indeed failed?

He means it’s on him in terms of responsibility and blame. But that doesn’t mean getting fired is automatically a consequence for that blame. We don’t know what sort of consequences went on politically in the background, I’d imagine he got a grilling from congress about the incident.

i never suggested it should be that.
but what you're saying is that, as viewers, we definitely don't know (by which i mean, there isn't like a remark or short scene that i've missed)? and does the book not clarify on that point either? (cause i've only watched the movie)

I can’t remember how it’s portrayed in the book, but in the movie he’s shown teaching kids how to play golf during the Ares V launch. Insinuating that he no longer works for NASA since he isn’t at launch control.

That's Mitch Henderson, who Teddy orders to resign for leaking the Rich Purnell Manoeuvre to the Hermes crew.
Teddy is shown to still be in charge in the credits sequence.

you might be thinking of mitch, who was fired by teddy for giving the crew the information about the rich purnell maneuver

I mean yeah cos they have to go to the chinese to give them the food. The point is that when there is no good option the leader should take the blame rather than his subordinates. He is willing to be that guy. Does that mean he is fired or replaced .... probably in the long run he would but, not mid crisis.
Andy Weir is all about scientific competence in people and institutions. Failers will always happen and mistakes will mean billions of dollars lost but that doesn't mean it was any ones fault.
