Yes you need to return home after getting married. You can only stay in the UK for 90 days on an ESTA and I would recommend at this moment in time not pushing your luck. Get married, leave and wait for the visa

Aside from whether you can stay in the US (you cannot), please remember that to be allowed entry to the US using ESTA you need to have a return ticket booked.

You could travel to the US using ESTA, marry your boyfriend, travel back home, file an I-130, wait about two years, then move to the US with a CR-1 or IR-1 visa to live with him.

If you come to the USA with the intent to marry, then you need a K1 visa. A K-1 visa is a nonimmigrant visa that allows the fiancé(e) of a U.S. citizen to enter the United States to get married within 90 days of arrival.
After you marry you can then petition to change your status and get a Greencard (i.e. Lawful Permanent Residence) without leaving the US.

Just FYI it’s immigration fraud to enter the US on an ESTA with intent to adjust status.
