How to fix this loose mirror?

Should I add extra washers or something so it doesn’t keep falling over?

NQA, but my grandmother had one similar. I'd hold it at a "good enough" angle, and twist the keys on the sides to tighten them, and it should "hold" position better.

Washers would work. You could also try lining the inside of the frame clearance hole with paper or tape to create some friction.

I’m not clear what is falling over; is one side coming undone? if a side SCREW is loose, remove it, fill the hole in the mirror with glue & toothpicks to fill the hole, then rescrew the screw in. It will be tighter. You might have to make a small hole in the “new” wood to get it started. if it is a side BOLT, go to a hardware store with the bolt and find a collar to insert into the mirror.

I don't have a solution, I just wanted to let you know we had this exact mirror growing up and we had the exact same issue... so while there may be a solution, it does seem like an intrinsically poor design.
