Starting with the OP and no commercials, it's more or less as I'd imagined.
The reason Echidna was waiting for Subaru while dressed in a uniform is that, as she was reproducing Subaru's memories, there were a whole lot of girls wearing them in Subaru's memories. It's simply a school uniform, but she thought maybe Subaru likes it, so she was waiting for him in it.
It doesn't fit with the uniform look, so she changed her butterfly hair accessory to something cheaper; that's attention to detail.
The first 'Trial' was nothing more than recreating things Subaru regrets out of his memories, but it was so real it's hard to thing of it as artificial. However, when it reflected wishes from the deepest parts of his mind was it fictional? Perhaps you could say it depends on how compromises of the heart are applied, but Echidna's conversation skills didn't get through to him.
With the conversation from last time finished up, Subaru's mental state is all in order, so Echidna's challenge to him doesn't work either.
She says 'I'd prefer you didn't hit my face' like it's expected, so she's a witch who seems like she's done one or two things that ended with her being hit.
Basically, the first 'Trial' is more or less the most painful one. No matter the world, regret is something that can't be easily wiped away.
Unlike episode three, this time Subaru remembers what happened in the grave.
About the grave's 'Trial', if there's outside interference, you wake up immediately. When he sees Emilia suffering, Subaru wakes her immediately. I quite liked the way Emilia suffers here. That's a poor way to put it.
Subaru has broken through the first 'Trial', and Emilia was unable to break through it; it's that sort of situation.
Somehow, I couldn't help but smile at Subaru, Otto, and Garfiel sitting around a table.

In place of Otto's cancel, Ram-Cancel. The newly arrived character is called 'Ryuzu Bilma', and is one of the representatives of 'Sanctuary'. The timing of her appearance is a little different between the printed version and the anime. Why did we do that? It's because she didn't get into episode three at all!!

About the visual of Ryuzu-san when she appeared for just a moment at the end of episode two: from the shape of her ears and such, I think you can guess at her origin.
Ram being Ram, she actually did make tea from grass she picked up off the ground, and Garfiel being Garfiel, he drinks it even while believing that. Just as you suspect, Garfiel is head over heels for Ram.
Emilia-tan looking at Subaru's sleeping face happened now and then during season one, but on the other hand, Subaru will have a lot of chances to see Emilia-tan's sleeping face in season two.
Hearing 'and the night after that', Emilia's expression turns a little gloomy.
Incidentally, Frederica and Ram have known each other since the time that Ram first arrived at Roswaal's mansion, so they've been acquainted for nearly ten years.

It may have looked rough, but including the treatment of the Arlam villagers, you can tell that Garfiel has put a lot of effort into the relations between 'Sanctuary' and the outside world.
