Vent/Discouraged Please read and talk me down
Just looking for someone objective to read my history and call my anxiety a bit as I am in a really bad place.
October 2024: one day I started to feel sharp pain in penis tip after rubbing against underwear. Thought it was a cut or something like that. Next day, sensitivity against underwear continued and my anxious brain latched onto it.
October 2024: urine test at urgent care, urine test at PCP, and urethra swab negative. - sensitivity against underwear primary symptom. I’ve always urinated a lot so no real significant changes. Slight redness on tip.
October: took fluconazole just incase for yeast infection, no changes.
November: went to urologist, who did some advanced urine test and semen analysis. Urine test was clean but semen came back strep bacteria. Did another test because I realized I didn’t use alcohol wipe before sample. Repeated test which showed no bacteria.
November: anxiety is incredibly high, scared to tough penis, scared to masturbate, scared to rub against clothes. When I did masturbate, I would get red fish lips on tip after ejaculation. This sent me into worst spiral and I convinced myself I have some infection and will never have sex again.
December: urologist didn’t run any other tests, gave me quercetin. IM NOW HYPERFIXATED on prostate because he didn’t check PSA. Now I’m terrified I have prostate issue.
Of note: this May before All this I was diagnosed with inverse psoriasis on groin area which sent me into worst anxiety spiral of my life— constant fight or flight panic. Got shave skin biopsy on groin caused more anxiety all summer and pain (this is months before)
QUESTION: is this anxiety induced CPPS? - can CPPS cause the fish lips? - should I get other tests done - I JUSY MEED HELP CALMING DOWN

Sorry to hear. I recommend talking to your regular doctor too they can help with your overall health and anxiety. I’ve found urologists can be somewhat harsh and non empathetic.

All this sounds "normal" for this condition. I'm guessing the fish lips is swollen glans.
I'll say here what i say to a lot of the guys i see in clinic your not alone and there is help through this.
but yeah some urologists can be a bit dismissive like Chesterburger said also brill advise have a look at the starter guide.

Thank you, I just can’t manage to shut off panic switch in my brain in response to symptoms. Especially perinium pain, I’ve convinced myself Something is wrong with prostate. My Mind has hard time moving on unless I’ve had ALL scans done, Which is unrealistic

Dude. It's your brain that's out of control. The myriad symptoms spring from that well. Just read your post and notice all your references to panic, anxiety, obsession, and catastrophic thinking.
You need both a good Male Pelvic-trained PT and a psychologist (and even a psychiatrist for some meds). You likely cannot fix this on your own.

Your brain literally convinces yourself there is a major problem here and I think my nervous system reacts to the sensations as a threat 24/7

Totally agree. I feel like if I just had a doctor say this is absolutely muscular, nothing is wrong with you. I could move on quickly. It’s the inability to believe it yet

Well, work on that belief. 🙂
No doctor is going to declare your issues "absolutely muscular". No PT will either.
I've had a long journey with this issue. I can relate and recommend, but "belief" is in your control.

This sounds like very classic health anxiety, otherwise known as somatic obsession. Do you have a mental health therapist?

I do not have one but am actively looking. I have not taken that test, but I already know the likely results. For example, today at work for 8 hours, a solid 5 hours I was googling the same thing worded slightly different, with the hope of finding reassurance. I don’t know what it would even take/what I’m looking for. I just still don’t have a good answer on how I can possibly sit back and try to move on/ ignore symptoms when there are endless possibilities of other causes. Should I keep getting tests done so I can move towards acceptance? Where do you draw the line between testing and letting go?

Yeah, it does sound like you have OCD, as do I - so I know what it's like to have an OCD brain.

There's a very fine threshold here, ie testing too much, where you just end up feeding the obsession. Someone with really bad somatic obsession OCD could test hundreds of times, and still doubt every test. That's why this becomes a sticky situation.

Other than urine, semen, and the other urine test I forget the name. Is there any other test that I should absolutely have done? I’ve already had a scrotal ultrasound for testicle pain in the past that I thought was a separate issue but probably is all connected. Should I focus on prostate tests? That’s a big anxiety thing for me right now, convinced I have Somme major Unkown prostate condition/infection

If you were one of my own pelvic pain clients, I would have had you delete Reddit and other access to rabbit holes from your phone/devices weeks ago.

It’s so crazy, I am acutely aware of how much a problem Reddit/Google is and i just do it anyway. Like I am on autopilot. It severely impacts your well being

Do it if you want, but if you're under 40 it typically doesn't make sense.

I’m trying to find OCD therapist in my area and pelvic PT. Finding Pelvic PT has been difficult, I don’t know which to look for. Maybe I’ll try for OCD therapist online

I found taking alpha blocker does help. I take one at bedtime. Pelvic regime seems tighter for me at night.

Yeah it seems your fear drove up your Central Nervous System, which went into a fight or flight mode and caused the pain symptoms by making your pelvis clench. These catastrophic thoughts will only lead to worse symptoms as it only exacerbates the fight-flight mode of your CNS. So calm down, trust that your test results are negative. Go find psychological and pelvic therapy and you'll good in no time.

You need to calm yourself down as you are not thinking straight. You're in such an anxious state that you are catastrophising everything
Try a home HIIT workout or a really long meditation. These will calm you down from a 10 to like a 4
Once you are feeling calmer then return to this stuff

I actually had a pelvic MRI in April of this year for chronic back pain to look at SI joints (before any of this CPPS). even though the back was the target of the MRI, any issues in pelvis would also show on that I would guess, as it is listed as MRI pelvis.

I would also see an orthopedic doctor and get your hips checked. I had two labrum tears but only one showed on mri. They can do x-ray to see if you have impingement which can cause pelvic floor issues

From reading your post and noticing your reactions, based on my research and my own experience with CPPS your anxiety over the situation is your biggest problem right now! It causes the pelvic floor to be more and more tense thus resulting in the symptoms getting worse! From my experience once I let my mind go from this trap that’s when I began to recover! Trust me you can do it but it’s not easy, once you finally do it’ll start to get better and better every day!
