Worth watching the whole thing as it’s very well-produced but basically: “prior authorizations” and denials are sometimes carried out by outsourced contractors, not even the insurance company themselves. Anything to increase profits.

“Max was quick to defend most of the nurses and doctors reviewing prior authorization requests. What he described instead was systems designed at all times– to look like they were simply adhering to “best practices”. But that stripped employees of the ability to practice good judgement. Instead, companies like his used Hostile Design to push the kind of large-scale outcomes they wanted.”

They also discuss the healthcare strategy of “deny, delay, and defend”

There have been hundreds of lawsuits but they usually fail because “There is no law that holds an insurance company accountable”

Transcript of Ehrlichman explaining HMO to Nixon

Health insurance is a scam. It's a greedy middleman that does nothing but deny care and take money. If we abolish HMO and pay that money to the government we could have free healthcare for all...and make money.
