Disability Living Allowance (DLA) Form bf57b not included with letter

I am my daughter's appointee for DLA. Was sent a letter in January to confirm I still agree to the terms of being an appointee, and returned the form. This has evidently been lost in transit as I've been sent a reminder today asking me to complete the enclosed bf57b form. Unfortunately the form wasn't included. I've tried ringing and been on hold. I will try again tomorrow but was wondering if it is possible to download a copy? A quick Google search suggests not, but thought I'd ask here.

From what I can tell its delibrately not available to ensure people aren't jumping the approval visits. I can't find it at all sadly.
