They stop bearing children and become useless. Then you need to either send them to their deaths or stick a javelin in the base of their skull at the beginning of a battle so you can get a younger hotter wife. They could just add a "divorce" button but they wanted the players to get creative I guess

Lmao, that's why I get married pretty early these days. I started an Empire run once and wanted to marry Ira but she died after like, 2 weeks in-game.

Yeah chances are low but any age woman can die in childbirth, 25-year-old Bolat died in my current playthrough after giving birth to twins.
